In what cases is stimulation needed and when it is not carried out

How is the stimulation of ovulation with Clostilbegit

Stimulation of ovulation with Klostilbegit is widely used in medicine. The active substance of the drug Clostilbegit, produced in the form of tablets of 50 mg, is clomiphene citrate, which binds estrogen receptors in the hypothalamus. Such treatment is allowed for no more than six menstrual cycles. The maximum amount of the drug consumed should not exceed 150 mg per day. The second method of drug administration implies, on the contrary, a gradual decrease in dosage. The starting dose for the first week is very high, 100 - 150 IU. This option of taking Gonal-F is recommended for women over 35 years of age, as well as for irregular periods, small ovarian volume, after surgical intervention on the internal genitalia. However, this method of stimulation is rarely used in practice, since it requires a lot of experience and high qualifications of a medical specialist.

Stimulation of ovulation with Letrozole

Letrozole was originally used to treat menopausal women for breast cancer. Today, the drug is used as a highly effective ovulation stimulator. Often it is prescribed to replace Klostilbegit. The drug is produced in the form of tablets. The stimulation process begins on the third day of the menstrual cycle, the course lasts 5 days. The daily dose is 5 mg, that is, two tablets. Compared with Clostilbegit, the drug is less powerful, its effect extends for a short time, but side effects are recorded infrequently.

Stimulation of ovulation based on hCG

Stimulants based on chorionic gonadotropin have the action of luteinizing hormone produced by the pituitary gland. Data medicines are used at the second stage of ovulation stimulation, accelerate the process of rupture of the follicle and the release of a mature egg from it, participate in the preparation of the mucous membranes of the uterus for the introduction of the zygote. The use of the drug is possible only if the thickness of the endometrium in the uterus is at least 8 mm, and the diameter of the dominant follicle in the ovary reaches at least 18 mm. It is necessary to enter chorionic gonadotropin every three days, 1500 IU. The course lasts 10 days. Ovulation usually occurs two days after drug administration.

Stimulation of ovulation with dydrogesterone

Dydrogesterone is produced in the form of tablets of 10 mg. In pharmacies, it can be found under the name Duphaston. The drug has a progestogenic effect on the uterine mucosa. The stimulant is used in the second half of the menstrual cycle. The daily dose is 20 mg. The duration of the course is at least 18 days. For greater efficiency, the combined use of Dydrogesterone and Pregnil is allowed.

Stimulation of ovulation with vitamins

What is anovulation?

Lack of ovulation (anovulation) is a violation of the growth and maturation of the follicle, as well as the absence of the release of an egg from the follicle. Such deviations can occur in women, both with a regular cycle and with its failures.

Recall the sequence of the menstrual cycle. It is divided into two phases: follicular and luteal. The first prepares the body for conception, the second - for pregnancy. These phases are controlled by special hormones produced by the pituitary gland - gonadotropins: follicle-stimulating (FSH) and luteinizing (LH) hormones.

The "conductor" of the first phase of the menstrual cycle is FSH. At the beginning of the cycle, the concentration of this hormone is maximum. It stimulates the maturation of the follicle, inside which a young egg is preparing to meet with the sperm. As it matures, the follicle increases in volume and produces an ever-increasing amount of estrogens (including the hormone estradiol). For the pituitary gland, an increase in the concentration of estradiol is like a trigger: it reacts with a strong release of luteinizing hormone. Under the influence of LH, the follicle bursts and ovulation occurs. The residual follicle turns into a corpus luteum and begins to produce progesterone and wait for pregnancy.

Difficulties with ovulation begin with insufficient production of gonadotropins, when the follicle does not mature or there is no egg in the follicle, and also in cases where the egg cannot leave the follicle due to a too dense shell. Unfortunately, anovulation is one of the most common causes of infertility today.

The main effect of these drugs is to increase the level of FSH and LH. However, the appointment of antiestrogenic drugs should be approached with great caution: the slightest violation of the dosage can lead to hyperstimulation, which threatens polycystic ovaries, their depletion and early menopause. In addition, it is recommended to conduct no more than six courses of such therapy in a lifetime. And, therefore, the use of these funds should be carried out under the very strict supervision of a physician.

    A milder scheme for influencing the ovaries is considered to be treatment with injections based on human menopausal gonadotropin(for example, Menogon, Pergonal, Menopur). These drugs are synthesized from the urine of menopausal women. They contain the hormones LH and FSH, which are responsible for regulating the menstrual cycle and maturation of the follicles (and therefore help trigger ovulation).

    The most effective and safe today are considered recombinant FSH(Puregon, Gonal-F). These drugs are the most expensive. They are received in laboratory conditions by genetic engineering, they are practically devoid of impurities and as close as possible in their action to the natural hormone FSH.

    In some cases, injections of drugs containing human chorionic gonadotropin (hCG)(for example, Ovitrel, Pregnil, Horagon, Prophase). Such a drug is used in a situation where the follicles independently mature and develop, but then regress and do not release an egg, as well as in IVF and AI schemes. HCG stimulates the rupture of a mature follicle, which releases an egg ready for fertilization into the fallopian tube.

Stimulation can be carried out with antiestrogenic drugs and gonadotropins. Antiestrogenic drugs at one time deservedly received priority, because then there were no good offer by gonadotropins.

Usually, they begin to stimulate ovulation with antiestrogen drugs (if the effect is not obtained, they switch to gonadotropins). Antiestrogen drugs are cheaper than all other stimulation drugs. But they have a number of negative properties. Firstly, they have a negative effect on the endometrium (and when pregnancy occurs, the endometrium should be very good). For example, there is an opinion that urinary gonadotropins are more suitable for those patients who are already over 35 years old. In general, gonadotropins are more effective than estrogenic drugs.

HCG injection is done once during stimulation (within one cycle). HCG - included in therapy with gonadotropins (or estrogens).
